EPC provides specialist palliative care to people with a life limiting or terminal illness in the last year of life. 

EPC services can only be provided to people who live in one of the following areas:

• Boroondara
• Manningham
• Maroondah
• Whitehorse
• Monash
• Knox
• Yarra Ranges

If the person you are caring for falls into both of the above two categories and you would like to refer them to the EPC program, please click below to complete a Referral Form. Alternatively, you can phone 1300 130 813. Please note that you can only refer someone if they have consented for you to do so.
