I Need Palliative Care

EPC provides specialist palliative care to people with a life limiting or terminal illness in the last year of life.

EPC services can only be provided to you if you live in one of the following areas:

• Boroondara
• Manningham
• Maroondah
• Whitehorse
• Monash
• Knox
• Yarra Ranges

Re-referral

This form is to be used for people who have previously been referred to Eastern Palliative Care.
